There’s a huge mystery that seemingly doesn’t get resolved after the final boss in The Phantom Pain. If you’re confused and want answers, it’s possible to complete the circle and see what really happened in that hospital. There are other optional endings to individual character story-lines too, and we’ll cover how those are unlocked with the guide below.

‘The Truth’ & Extra Endings Guide
After completing Episode 31, Chapter 2 of The Phantom Pain begins with optional high difficulty missions alongside new required missions.
To reach Episode 45 (Quiet’s Ending) or Episode 46 (True Ending) you’ll need to complete these optional and required missions. Optional missions are denoted with a white circle while important required missions have a yellow circle.
Episode 45 – A Quiet Exit
To get this mission, you must complete Episode 43 – Shining Lights, Even in Death. Earn Bond Level 10 with Quiet and a new important Side Op will appear in Afghanistan.
Investigate Side Op – Secure Quiet and be prepared. After scanning the intel you’ll immediately start the Episode 45 mission with no way to exit once it begins. Bring a powerful Launcher weapon.
Episode 46 – Truth: The Man Who Sold The World
The truth is revealed after completion of Episode 43 – Shining Lights, Even in Death. In the post-game, complete any important Side Ops or optional missions until these are unlocked.
Not every mission must be completed — all but four main missions are required to unlock Episode 46. This means you can skip any [Extreme] boss battles. All important main missions up to Episode 43 are required, but Episode 45 is not required to unlock this ending.
